Client Patient Access Services (CPAS)

Client Patient Access Services (CPAS) provides assessment and care coordination to help individuals access appropriate community-based health and support services.

CPAS supports connections to services such as:

• Home Care

• Long-Term Care

• Permanent admissions or respite services

• Physical Therapy

• Occupational Therapy

• Social Work

• Other community and health supports
  
  

Referrals to CPAS may be made by anyone, including clients, physicians, family members, or caregivers.
